Part II

Complete the application by mailing the following information to the selection committee (address provided at the bottom of the page). Part II of the application should include:

• A formal nomination letter that includes why the work is worthy of consideration;
• Six copies of the article, report, or book (these will not be returned);
• A short biography of the author(s) including current affiliations and achievements; and
• Two official letters of support from independent references other than the person submitting the nomination (can be sent separately).

All parts of the application should be received by June 30, 2006.
